Two separate services
Form & Function provides the coaching purchase described on each program page. Evolv handles any requested clinical assessment. RealTimePay, Inc. is not the prescriber or pharmacy and does not determine medical suitability.
Semaglutide and tirzepatide
These are separate prescription-treatment options to discuss with a clinician. A coaching selection does not prescribe, combine, supply or guarantee either medicine. The clinician decides suitability and explains benefits, risks, alternatives and any monitoring. The pharmacy completes its dispensing checks.
Costs and fulfillment
Medicines, consultations, laboratory tests and pharmacy services are not included in the coaching price. Any clinical offer must identify its provider, medication, price, eligibility and cancellation terms before you agree. Nothing is shipped merely because you buy coaching, receive an NFT or select a follow-up preference.
Your information
Use the clinic’s appropriate intake process for medical information. Do not submit medical records through our checkout or email. For questions about your coaching order, contact support@rt.app.
